INSTRUCTIONS TO DEBTOR AND COUNSEL ON CORRECTING INACCURATE SOCIAL SECURITY NUMBERS
[Applicable only to cases where 341 notice has incorrect number)
You submitted with your bankruptcy petition and/or Form 21 Statement of Social Security Number
an incorrect Social Security Number, or an incorrect number was entered when the case was
opened electronically. As a result, the notice of the meeting of creditors contains an incorrect
Social Security number. This error may make it difficult for creditors to identify the debtor's
account, and can adversely affect someone else's credit record. You are required to correct this
error and make sure that all interested parties are notified of the correction.
Within
10 days of today's date:
1. If your original Statement of Social Security Number (Form 21) is incorrect. submit an
amended Statement of Social Security Number (Form 21) to the Bankruptcy Clerk showing
the correct Social Security Number and complete steps 2 and 3.
2. Also, if the error involves the last four digits of your Social Security number, file an
amended bankruptcy petition with the Bankruptcy Clerk.
3. Serve a notice of the correction (and a copy of the amended petition, if necessary) on all
creditors listed on Schedules 0, E, and F, and also on the United States Trustee and the
trustee assigned to this case. File a truncated or redacted copy of the notice. showing only
the last four digits of the Social Security number. and a certificate of service with the
Bankruptcy Clerk.
FAILURE TO COMPLETE THESE STEPS MAY PROMPT THE UNITED STATES TRUSTEE TO
SEEK DISMISSAL OF YOUR CASE.
